Normal Singlife Account Opened more than 6 months, what are the terms and condition to claim for Retrenchment Coverage?

Currently I am curious if anyone actually managed to claim the Retrenchment Coverage from Singlife and what is the step by step process or procedure am I suppose to do if I need to submit a claim... Is there any min. amount needed in my singlife account? And am I suppose to use singlife card and spend in order to claim? Or it is not required?

See attached image for Retrenchment Coverage. The claim conditions are:

Retrenchment coverage limited to 1 year from the start of your SingLife account.
Retrenched and remained unemployed for 4 months.
Submit claim within 90 days from 4-month unemployment.
Claim amount based on average monthly card spend over 6 months before retrenchment.
Receive retrenchment benefit for 3 months, total cap at $10k.
